I am not familiar with 74HC138, but looking at data sheet
http://www.nxp.com/documents/data_sheet/74HC_HCT138.pdf looks like you need to move inverter from E3 (which you called E11) to E1 as per truth table.
I’ve attached the fix.
As just a style note you can provide notation in input and output pins – just a couple characters text to show next to the pin when the circuit is used on other one.
Anyway thank you for sharing your work here.
